C. The production number on your carrier, which can be found on the warning label on the inside of the waist belt or tag. It will look similar to SC-4AB (157) May. 1, 2016.
D. The unique serial number on your carrier, which can be found on a separate label on the inside of the waist belt. It might look similar to 112016-CIP 55955.
E. A photo or scanned copy of the original purchase receipt for the product as proof of purchase.
F. A digital photo or video of the broken or affected area
G. A brief description of what was going on at the time when the issue occurred
H. Current physical shipping address
